This is a typical circuit of the instrument amplifier AD8221. OP27 in the figure is used to set the reference voltage of AD8221. The output signal of the instrument amplifier is taken from the OUT pin and REF pin, which makes the AD8221 set up time short and improves the signal processing speed.

Look at the circuit should be the output reference voltage.
