Try increasing the resistance of R19 and reducing the resistance of R18.

85:18 When the output is 16V, the reflected voltage=17*85/18=80.

This is a very low reflected voltage. Normally, the reflected voltage we design is 90V, but yours is still lower than 90V.

Adjust the output sampling directly and increase it to 16V. Don't worry. According to your drawing, the R of RCD adopts 1M. I don’t know what the power is. Generally, the resistance of 5-10W is about 100K. If the resistance is too large, the RCD function is too weak, which may cause poor anti-peak absorption and lead to overvoltage.

Just adjust the ratio of R18R19! The most stupid way is to replace R18R19 with a potentiometer. After adjusting the output voltage you want, measure the resistance on both sides of the potentiometer.
